1. Mouthguard - MANDATORY FOR ALL GAMES
2. Football Cleats

Player Registration Checklist:

All documents must be filled out every year!

1. Player Registration form - please complete a registration form online every year. 

​2. Physical - this is the  IHSAA physical form which must be completed every year for all athletes participating in the King's Crusaders football program. Please complete the physical and return the form. 

3. Concussion Information - this is information from the IHSAA and Indiana DOE regarding concussions and sports. Please read the information sheets, print the signature form, sign and return the form.

4. Sudden Cardiac Arrest (SCA) - this is information from the IHSAA and Indiana DOE regarding Sudden Cardiac Arrest and athletes. Please read the information sheets, print the signature form, sign and return the form.

5. Player fees - please pay online or submit a check in person or through the mail. The address is listed on our contact page. 

6. Medical and Media Release Form - this form (pdf download below) allows the coaching staff to procure medical treatment for the athlete in the event a parent cannot be reached. It also includes a media release to use player images on our social media and informational brochures. Please sign and return the form.
